We collect the information needed to respond to your consultation request. Please keep medical details out of the website form.
What we collect
The consultation form asks for your name, email and/or telephone number, town or city, preferred contact method and permission to respond. We also record a request reference, submission time and contact status. We do not request symptoms, diagnoses, medical records or payment information.
How we use it
EyePRP uses your details to contact you about your enquiry, explain the referral process and arrange a suitable next step. Requesting a consultation does not subscribe you to marketing. We will discuss any practitioner referral with you before sharing your contact details with that practitioner.
Where information is held
The website is hosted on our Hostinger server in the United States. Enquiry contact details are encrypted in private storage outside the public website and accessed through an authenticated admin inbox. Email alerts contain a request reference rather than your contact details. Hosting and email providers process information to operate these services.
Cookies and security
Essential session cookies support the consultation form and secure admin access. We do not use advertising cookies, third-party analytics or embedded tracking videos. Short-lived, hashed network identifiers help prevent automated abuse. The hosting server may also keep standard security and access logs.
How long we keep it
Website enquiries have a 90-day retention period and are removed on the next routine intake or admin activity after that period. Security rate-limit identifiers become inactive within 24 hours and are removed during routine cleanup. Information needed for an ongoing relationship may be retained separately for that purpose. Practitioner medical records are handled by the treating practitioner under their own privacy practices.
Your choices
You can ask us to stop following up, request access to your enquiry information, correct it or request deletion by emailing info@eyeprp.com. We may need to verify your identity before responding. Do not include medical records or sensitive health details in ordinary email.
